You can file on the San Jose 311 website, in the mobile app, or by calling 311. An account lets you track a request, but the city also allows anonymous reports. A photo is optional. The location is not: give the closest address, cross street, or map pin so the crew knows where to look.
The same page has a My Home Services lookup for trash, recycling, street sweeping, water, and other providers tied to an address. That matters because the company or agency serving one San Jose address may not be the one serving another.
Do not use 311 for a life-safety emergency. For a routine report, say what happened, where it is, and whether it blocks a sidewalk, street, driveway, or other public space. Save the request number if you want to follow up.
